The Wall Street Journal reported that this possibly suggested an engine or hydraulic failure during takeoff, a rare event in commercial aviation.

Expert pilots suggest this engine failure theory is not likely as there was no evidence of yaw in the plane's flight, which would have definitely occurred had one engine been bad. The fact is that immediately after takeoff the landing gear should have been fully retracted and the flaps fully extended. Instead, the landing gear was fully extended and the flaps fully retracted. No airplane of that size, weight and wing design can fly under that configuration.

It’s painful to think that pilot error led to this.
